Clinical Care Visits provide members with a personal visit by an Altegra Health Licensed Practitioner, who will conduct an in-home wellness evaluation. These visits allow members to obtain comprehensive health assessments, medication review, home safety check and lab tests, if clinically indicated.
Quality improvement interventions such as clinical care visits allow healthcare organizations to have a greater, positive influence on the health of their members. Through Altegra Health’s clinical care visits, members receive a personalized care plan and are encouraged to review results with their physician. The resulting clinical documentation is then processed via claim to the health plan, generating data for risk adjustment and HEDIS.
- Provider Recruiting and Credentialing – Recruit and hire locally available, board certified, licensed practitioners, NPs, PAs, MDs and DOs. Be confident that each provider participates in rigorous peer-to-peer training prior to conducting visits, including documentation requirements to meet coding guidelines.
- Comprehensive Assessment – Meet criteria for Medicare’s Annual Wellness Visit including: history and physical, medication review, Katz ADL, fall risk assessment, nutritional assessment, home safety review and depression screening. Lab tests are provided based on health plan contract and clinical indications.

Manage progress and pace of the outreach, including visibility into unreachable members and all details to conduct additional research. View and download all documentation, including the assessment, lab results, follow-up care recommendations and escalation details.
